The Perioperative Care Fellowship is a new post which has been developed to maximise the training opportunities associated with the development of the new Enhanced Perioperative Care (EPOC) service at the Queens Medical Centre. The EPOC project has been running for a year with a new unit completed at the QMC in early 2023. Perioperative care is also being developed at NUH with a new Prehabilitation program running since 2022. In addition to opportunities associated with the development of this service, further clinical opportunities include:
Mentored sessions in high-risk Preoperative Assessment Clinic.
Training in cardiopulmonary exercise testing and interpretation
Involvement in exercise prescriptions for patients entering a Prehabilitation program
Supervised operating lists in major colorectal, hepatobiliary and head and neck surgery, with others at request.
Experience in multidisciplinary high-risk planning meetings.
Opportunities to attend cardiology and respiratory clinics including sleep clinics.
Opportunities to attend specialist postoperative ward rounds.
Full support to meet the 2021 curriculum stage 3 specialist areas learning outcomes for perioperative medicine.
Quality Improvement and academic opportunities
The post-holder would be involved in promoting the new service at the QMC and have the opportunity to use this experience cross site in assisting Nottingham City Hospital develop their EPOC service. They would be responsible for developing guidelines and policies and lead Quality Improvement projects in perioperative care. This could also include involvement in the new NUH Prehabilitation project establishing links between patient’s preoperative exercise program and their care on the EPOC unit. There would be an expectation that the candidate will involve the Queens Medical centre with PQIP: improving outcomes after surgery. In addition, opportunities will include, but not be limited to:
Delivering and developing education in pre -operative assessment and perioperative care to multidisciplinary personnel.
The appointee will take part in regular departmental academic, audit and governance meetings.
The post-holder would be expected to participate in Continuing Medical Education to the level required by the Royal College of Anaesthetists
Supervision
Supervision will be mostly level 1 (Consultant Anaesthetist immediately available in the same theatre), and sometimes will be provided within the theatre suite (level 2).
Educational supervision will be provided by Dr K Corrie (EPOC) and Dr D Evans (Preoperative assessment/Prehabilitation). Research/QI supervision will be provided by Professor B Baxendale. Weekly working patterns and learning objectives will be agreed at the outset and regularly reviewed. Outcomes will be assessed by review of logbook and reflective portfolio.
Job Plan
Weekly Hours: Approx. 48 per week
On-call: Resident on-call for anaesthesia joining the pool of senior SpRs, covering theatres, trauma calls - major trauma patients in ED department, the obstetric anaesthetist on call, and providing support to the ITU trainee. (approx. 1 in 7).
